🩺 My Medical Information Kit
The Zion Fire & Rescue Department is proud to offer FREE My Medical Information kits to senior citizens, residents with special needs, and others in our community who may benefit from them. These life-saving kits are funded by Advocate Condell Medical Center and sponsored by the Lake County Fire Chiefs Association.
What’s Included in the Kit
Each kit consists of a bright red magnetic pouch designed to attach securely to your refrigerator. Inside you’ll find a medical information form where you can record:
- Personal identification details
- Emergency contacts
- Current medications and allergies
- Medical conditions
- Advanced directives or DNR information
A special sticker is also provided for placement near your home’s entrance. This alerts first responders that you have a My Medical Information kit available, ensuring paramedics can quickly access vital medical details during an emergency.
📦 How to Get Your FREE Kit
To request your kit, please contact our Administrative Assistant at (847) 746-4038 Monday – Friday, 9:00 AM – 5:00 PM. Provide your name and mailing address, and we’ll mail your kit directly to you.
🚶♂️ You may also stop by Fire Station 1, 1303 27th Street, Zion IL to pick one up in person.
