Citizen Complaint Form

This form allows residents to communicate with City personnel regarding questions, concerns, or complaints.

Please note: For a complaint regarding the Police Department, a separate form must be completed and submitted to the Zion Police Department.

Police Department Citizen Complaints

If you witness an officer acting outside of our mission or values, please complete a Citizen’s Complaint Form. After notarizing the form, submit it to the Zion Police Department. A notary public is available on-site Monday through Friday, 9 AM to 5 PM.
